Please fill out this form if you plan to play 3rd/4th Pink, 5th/6th Pink or 7th/8th Pink this Spring. If you are looking to play on the advance Purple Level, you will need to attend a Skills Placement Session BEFORE you fill out the Player Information Form.

All IGLA parents and players must agree to the terms and conditions of our WAIVER in order to play in any program. Please click on the link above and read our waiver carefully. In the following step you will be asked to check the box acknowledging the Terms of IGLA Waiver have been accepted.

IMPORTANT! The IGLA Player Information Form gives your coach super duper important information about you and your daughter. This form includes uniform sizing, teammate requests, and other key information to help with IGLA's communication to YOU!

*This form is required for all participants. Chicago, Elmhurst, Evanston, Kenilworth and Lake Zurich participants: this is your only registration form and will direct you to payment options.
All other locations will fill out the form below, but submit payment directly to their participating Park District: we still need to collect information like waiver consent, uniform size and teammate request. Please note: You can only sign up through one Park District, registering through multiple Park Districts is not allowed.
